NARRATIVE

Analyst met with Director Shelli Harder who advised him that the facility was closed for a one week break but allowed analyst to inspect the Center's indoor and outdoor areas and review staff and children's records. Analyst inspected classrooms A/B, 1, 2, 3 and 4. Furniture, supplies and equipment were found to be in good repair. The facility has adequate heating, lighting and ventilation. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathrooms are lighted and have ventilation. Outdoor equipment is age appropriate with shade and sand cushioning underneath. The last fire and earthquake drills were conducted on 04/16/19. Drinking water is available in the classrooms and outdoor play area. Analyst reviewed medication policy, storage and facility IMS plan. Nine children currently have medication that is administered at the facility on an as needed basis. Medication is kept in director's office where it is inaccessible to children but is immediately available for use in an emergency. Analyst verified today that at least two staff members have current CPR & First Aid certificates. Isolation area for ill children is in the office. A review of staff records on indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances. Five children's and five staff members records were reviewed today. Sign in/out sheets and food service were reviewed. The license, snack menus, parent's rights, personal rights, earthquake preparedness checklist, emergency disaster plan, floor plan and car seat law are posted in a prominent area. Director advised analyst today that current teacher Sharon Halle will be taking over as director for the facility in September and provided him with the director submission packet for final review and approval before she begins her duties. Analyst will update director change in system when new school year begins in September. Director provided analyst with copy of facility roster and personnel report during today's visit.

Incidental Medical Services policy was reviewed with site supervisor.
