On September 6, 2022, at 12:50 pm, Licensing Program Analyst (LPA), Jessica Rubio conducted an annual inspection as part of a compliance review. LPA met with Director Kim Piech. A tour of the inside and outside of the facility was granted and the following was observed and/or noted:

· The following items were posted and updated where necessary:
- License
- Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148)
- Parent’s Rights Poster (PUB393)
- Personal Rights (LIC613A)
- Child Car Seat Law
- Snack Menu
· The facility is operating within the limits as stated on the license.
· Ratios are being met during this inspection. Adequate supervision was not being provided during the inspection. LPA observed two children at different times going to the bathroom outside of the classrooms on their own without supervision. A citation will be issued.
· Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards
· There are no weapons present at the facility as stated by Director Kim Piech.
· LPA observed four small plastic kiddie pools on the playground area. Each pool had approximately 2-3 inches of water in it. Director immediately emptied the pools. A citation will be issued. All wading pools or similar products must be emptied immediately after use and stored in an upright position.
· Drinking water is provided in the indoor activity space by filtered water supply and in the outdoor activity space by filtered and public water supply.

· There are no medications stored at the facility at this time.
· Hazardous items are not stored where inaccessible to children. LPA observed bleach being stored in the kitchen area on a low shelf accessible to children. The door to the kitchen was not locked. A citation will be issued.
· All floors were observed to be clean and safe.
· Bathrooms were observed to be safe, sanitary and in operating condition
· Playgrounds are enclosed by appropriate fences and free of hazards
· Outdoor activity areas are supplied with age and size appropriate equipment in good condition
· Food preparation area is clean, free of litter and rubbish and free of rodents and other vermin
· Food is stored appropriately and protected from contamination
· All storage containers for solid waste were observed to have tight-fitting covers that are kept on, and in good repair
· Sign in/Sign out record was reviewed and meets regulation requirements
· Disaster drills are conducted at least every six months – last drill conducted on 8/26/2022.

A review of eight staff files and ten children's records were conducted as part of this evaluation.
· Children’s records were found to be complete during this inspection.
· During record review, LPA observed that three staff (Ref#1,2 and 5) did not have record of immunizations. Four staff (Ref #3,6,7 and 8) did not have current mandated reporter training on file. Three staff (Ref# 3,5, 6) did not have health screenings on file. Citations will be issued.
· Staff records review indicates that four staff (Ref #2,4,5, and 6) present did not meet minimum qualifications for the position for which they were hired. A citation will be issued.
· A staff member is present with current Pediatric CPR/First Aid which expires on August 2023
· Opening and closing staff member’s CPR/First Aid expires on August 2023

· Director completed Health and Safety Training on 10/18/2015.
· A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following items were discussed with the Director during inspection:


· Menus shall be posted at least one week in advance in a place visible by the child’s authorized representative, dated and kept on file for 30 days, and made available upon request.
· The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall
· The Licensee was informed of their reporting requirements and provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO10@dss.ca.gov
· The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations858@dss.ca.gov

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The facility is being cited for California Code of Regulations Title 22 Regulations and Health & Safety Code Sections. See LIC809-D for cited deficiencies.

An exit interview was conducted, and this report was reviewed with the Director Kim Piech. Appeal rights were discussed and provided during the exit interview. An LIC 9224 was provided to Director as well. Signed copies must remain in children’s files for 12 months. A notice of site visit was given and must remain posted for 30 days.
