On 03/02/2022 at 9:00am, Licensing Program Analyst (LPA), Selina Siao conducted an unannounced visit to deliver the above allegation finding. The initial visit was conducted on 01/12/2022. LPA met with Director, Donna Taconi to conduct a tour of the facility. LPA observed 3 out of 92 that were inside their classrooms or lining up to go to the playground not wearing a mask. Analyst observed children wearing mask during indoor drop off. Children are not required to wear mask until they go to an indoor area. Signs are posted throughout the facility regarding the mask mandate. Appropriate staffing ratio is observed, and all staff members have the required background clearances and are associated to the facility.
Throughout the course of investigation, interviews were conducted with Director and several day care parents. Information obtained from interviews were contradictory to the allegation. Based on information obtained and LPA's observation, the allegation is determined to be unsubstantiated which means although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation occurred. No citation issued.
Appeal Rights (1/16) were discussed and provided. Facility was advised to post the Notice of Site Visit and that failure to keep it posted will result in $100 civil penalty.
