13 | Licensing Program Analyst (LPA), Joelle Redding, made an unannounced visit to deliver complaint findings on the above allegations. During the investigation, LPA observed the operation of the facility, interviewed all facility staff to include the Director, and obtained feedback from a large sample of parents of enrolled children. A sample of children were also interviewed. LPA did observe that there was a unsecured drawer in the kitchen that contained sharp knives when touring the kitchen and observed that there was a swing that had been in disrepair and had eventually broken while a child was on it. No injury was sustained. In addition, information obtained during interviews of staff and children indicated that there were times when the older PreK children were allowed to walk down the hall to the bathroom without direct supervision of a teacher, most often when there was just one teacher in the PreK classroom. Based on LPA’s observations and interviews, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED, California Code of Regulations, (Title 22, Division 12 & Chpt 1) are being cited on the attached LIC 9099D. Appeal Rights (1/16) were discussed and provided. Signature at the bottom of this report confirms receipt.
