25 | On 09/27/2023 at 3:05pm, Licensing Program Analysts (LPAs) Selina Siao and Sherlynn Banas conducted an unannounced inspection to follow up a self-reported incident that happened on 08/29/2023. On 08/29/2023, one of a child in the two-year-old class had taken off her shoes while playing at the two-year-old playground. The two-year-old child started crying so the teacher (T1) went to the child and noticed that the child's feet were red. Staff put water on the child's feet and put the shoes and socks back on the child. The co-teacher (T2) of the class was informed and during the child continued to cry so the T2 took off the child's shoes and sock and noticed that the child's feet near the toes looked red and blistered.
Interview was conducted with staff members regarding the incident. Based on information obtained, the children in the two-year-old class children often likes to take off their shoes and sock off. Facility staff stated that in the future, they would bring the children inside the classroom if they refused to keep their shoes and sock on their feet to ensure the children's safety. The parents were notified as soon as the staff noticed the child's feet had blistered. The child continue to be in care.
The following census was observed during today's inspection. Discovery Pre-K class had 12 children supervised by staff Andrehya Azzato. Preschool 1 had 10 children supervised by staff Zarifa Alidost and Pre-K-1 had 10 children supervised by staff Laronika Linton. Appropriate ratios were observed during today's inspection.
