On 05/08/2023 at 9:10AM, Licensing Program Analyst (LPA) Selina Siao conduct an annual inspection. Facility operates Mon-Fri from 8:45am-11:45am. Upon arrival, LPA met with Director, Nancy Da Silva who was at the playground with 13 children (Giraffe Group) supervised by teachers Julie Ater and Cristina Lazaroiu. LPA proceed to tour the facility with Director to gather census. There were 15 children (Teddy Bear Group) in classroom A supervised by teacher Stephanie Salomon aka Veloso and staff Samantha Morris. LPA also conducted a tour of the other licensed day care areas including Classroom B (Multi-Purpose Room), Classroom C (Gymnasium), Classroom D (Lounge) and Gymnasium. Facility is within staffing ratio. All required notices, forms and license were posted. Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting and ventilation. Storage cubbies are readily available, and room accommodates class size. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Food service area consists of a kitchen which is clean and free of hazards. Snack menu is posted. Adequate food is available for snacks. Cleaning supplies are kept out of reach of children. Outdoor play area is a fenced playground with sufficient wood chips for cushioning. Climbing structures, swings and slides are securely fixed to the ground. Area has a large tarp used for shade. Director stated that the children only uses the smaller play structure. Children has their own water bottles that they use in the classrooms and at the playground. LPA reviewed sign in sheets, first aid supplies and reviewed medication policy and storage, all areas are within compliance. Isolation area is the Director's office. Personnel and client records were reviewed, and staff members have the required immunizations and completed the online mandated reporter training. All personnel have required criminal record and child abuse index clearances. LPA reviewed Emergency Disaster Plan and last fire drill was conducted on 03/07/2023. Facility has an operating carbon monoxide detector located in classroom A. Facility has at least one staff member that has a valid EMSA approved pediatric CPR/FA when children are in care.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Child Care Providers can now sign up for Quarterly Updates and PINS through the DSS website at https://cdss.ca.gov/inforesources/community-care-licensing/subscribe. LPA discussed California Megan's Law with provider and advised her to go on the website at www.meganslaw.ca.gov.

To improve the quality and value of the new inspection process, a survey will be sent to the email address provided. Please complete the survey and share your inspection experience. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The facility appears to be in substantial compliance. No citation issue. An exit interview was conducted, and appeal rights were provided.

A notice of site visit was given and must remain posted for 30 days. Failure to keep notice posted will result in a civil penalty of $100.00.
