On 4/13/2021 at 3:58pm, Licensing Program Analyst, Selina Siao conducted an unannounced case management inspection for a capacity increase with licensee via Face Time. Present at the facility is licensee and her mother/helper Adriana Juana Ontiveros. This is a 5 bedroom, 2.5 bath, 2 story home. The following day care areas were toured: day care room, 1/2 bath, fenced backyard and side yard gardening area. There is a fireplace in the day care room, and it is blocked off and there are bumpers around the hearth to ensure children's safety. The laundry room and garage areas are off limit and they are inaccessible by having doorknob cover to prevent children's access. There is a gate to prevent children to go into the off limit dining area, kitchen, front living room and another gate is at the bottom of the stairs to prevent children access to the upstairs off limit areas. Supervision is required when children are outside, and someone should be next to a child when a child is on the play structure or the swings. Licensee placed some soft rubber matting under and around all high climbing and play equipment to ensure children's safety. The fire extinguisher, smoke and carbon monoxide detector meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. Licensee stated that the home doesn't have any bodies of water or weapon. A review of staff records on this date indicates that all adults or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. The home appears to be large enough to comfortably accommodate a large license. Fire clearance was received on 04/07/2021. Licensee's has an EMSA approved pediatric CPR and First Aid certifications that are valid through July/2022. An updated roster was reviewed and obtained today.

The following information was reviewed with the licensee: information on reporting requirements for suspected child abuse and unusual incidents, children’s records, immunizations, adults living or working in the home and related civil penalties, shaken baby syndrome and SIDS. Licensee was reminded that corporal punishment, smoking, baby walkers, exersaucers, bouncy seats and jumpers are not allowed in day care.

LPA provided the handout regarding tips to prevent heat related illness to licensee today. The ABC’S of Safe Sleep: Sleep is Safest: Alone, on their Back in an empty Crib on a firm mattress.

LPA and Licensee discussed California Megan's Law and LPA provided: www.meganslaw.ca.gov.

Analyst read the report to licensee during the tele-inspection. A copy of the report will be e-mailed to licensee and she was advised that acknowledgement of the receipt of the report is to be received within twenty-four hours.

No correction is needed, a large license will be issue effective today.

The Notice of Site Visit must remain posted for a period or 30 days. Failure to keep notice posted will result in a civil penalty of $100.00.
