13 | Licensing Program Analyst (LPA) Jennifer Lott conducted an unannounced complaint investigation visit to deliver findings for the above allegation. LPA was greeted at the front door by Licensee's husband, Khan Rezaee and was granted entry after identifying herself and disclosing the purpose of her visit. There were no day care children present during today's visit. .
The Department’s investigation consisted of review of facility and outside source records, interviews with the licensee and outside sources as well as surveillance. It is alleged that during the month of November 2021, the licensee was operating over their licensed capacity. The daycare is licensed for a capacity of fourteen (14) children. Based on outside source records, interviews and surveillance, no children were visually seen entering, exiting or playing outside of the day care. Therefore, no evidence was found to suggest that the licensee was caring for children over their licensed capacity. This Department has found that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. An exit interview was conducted and report was reviewed with Licensee, Shekiba Rezaee. A notice of site visit was given and must remain posted for 30 days.
