1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Licensing Program Analysts (LPAs) Yolanda Baez and Martha Malane arrived at the facility to conduct a pre-licensing inspection for a change of ownership application. LPAs met with Applicant/ Director, Egle Athari. There were 20 children present at the time of the inspection. This Preschool program operates in the "North Classroom, South Classroom, East Classroom, and West Classroom". The hours of operation are Monday - Friday from 7:00am to 6:00pm. The "North Classroom, South Classroom, East Classroom, and West Classroom" was inspected and measured during this inspection and measure the following: North Classroom: Preschool Class (ages 2 years through Kindergarten): measured 354.14 square feet East Classroom: Preschool Class (ages 2 years through Kindergarten): measured 513.9 square feet West Classroom: Preschool Class (ages 2 years through Kindergarten): measured 257.27 square feet South Classroom: Preschool Class (ages 2 years through Kindergarten): measured 414.11 square feet All classrooms are equipped with appropriate furniture and napping equipment (cots) including tables, chairs, cubbies, bookshelves, and other materials for the children. The North classroom has direct access to a bathroom that is located inside of the North Classroom and the restroom consists of one toilet and one sink. There are restrooms available to the Preschool children located between the East and South classrooms and consist of 4 toilets and 3 sinks. There are a total of 5 toilets and 4 sinks available for the preschool use which is sufficient to accommodate the requested capacity of 43 children. Drinking water is available in all of the classrooms via water pitchers and disposable cups and children's sippy cups/water bottles (brought from home). There is one separate bathroom for staff located in the West classroom and is kept inaccessible through the use of a door knob cover. The staff bathroom is equipped with one toilet and one sink and will be used as an isolation bathroom for children who are ill. The Director's office will be used for the isolation area for an ill child. Disinfectants, cleaning solutions, and other hazardous substances are kept in locked cabinets. ...Continuation on page 2...

The kitchen area currently includes 1 sink, 1 stove, 1 dishwasher, a double oven, 1 refrigerator with a freezer, 1 microwave, storage space, and food supply. The menus are posted. The center will be providing electronic sign in and sign out via Kindertales Application. All required documents are posted on the parent's bulletin board near the Director's office. There is a carbon monoxide detector, smoke alarm, and first aid kit with required supplies.

The facility has a Preschool playground that is located towards the back of the building. The Preschool play ground measured 6,754.59 square feet, which is sufficient for 90 Preschoolers at one time. The Preschool playground is complete with age-appropriate play equipment, canopy and a large tree are used for shade, sufficient cushioning material, and is surrounded by a 4 feet high fence. Children have access to water outdoors via a water fountain with disposable cups and by taking their water bottles/sippy cups outside. Proper supervision and placement of staff on the play yard was discussed to ensure the safety of children at all times.

This facility plans to provide Incidental Medical Services – IMS and has submitted a Plan of Operation. Fire clearance was granted on 08/01/19 for a capacity of 43 children.

A license for 43 preschool children will be issued upon final review.
