On 11/10/22 at 12:30pm, LPA, Tyra Block, conducted an unannounced Case Management visit for the purpose of having the amended report from the inspection conducted on 8/17/22 signed. An allegation was not included in error.

Facility representative/ Assistant Director Soshia Garcia was present and signed the report. Andrea D'Agostino was absent.

Notice of Site Visit will be provided by email as LPA's printer has malfunctioned. The Notice of Site Visit must be posted for 30 days.
