25 | On 12/21/2020 at 9am, Licensing Program Analysts, Samantha Salunga and Annette Sutherland, conducted an announced Pre-Licensing inspection. LPA's met with Applicant/Director, Devorah (aka Divi) and her husband, Josef Fradkin. The School-Age Program will operate in the following rooms: SA1, SA2, SA3, SA4, SA5, SA6, SA7, Lab (L1), L2, L3, L4, L5, LH, LM, R1 R2, R3, R4, RL, Library, Art Room, and Auditorium. First Aid Kit and any medication is kept in the office. Fire clearance was granted on 11/10/2020.
All rooms were measured today at a total of 20,963.41 sq. ft. which can accommodate a total of 598 children. Outdoor play space is located in various areas within the campus, however, does not have a separate fence that surrounds each area. Age-appropriate supplies and equipment was observed available in all indoor and outdoor activity space. There are 23 toilets/urinals (which allows individual privacy) and 39 sinks available for the children. Toilet facilities shall not be utilized simultaneously by child of both sexes. There is a separate staff restroom which is also designated as the isolation restroom. The isolation area (due to COVID-19) is a tented area outside of the facility. The nurses office will also be utilized as an isolation area. There is a kitchen equipped with a refrigerator, stove and a microwave. Cleaning supplies and other toxics are inaccessible to children through latches, locks and/or placed on high surfaces. Drinking water is readily accessible via water bottles and refillable stations. Personnel files were reviewed during inspection to ensure that the the staff utilized as school-age teachers are qualified. Files were observed to be complete and qualified. Director was reminded to keep anything that reads, “Keep Out of Reach of Children” inaccessible to children.
