25 | On 01/14/2021 at 11am, Licensing Program Analyst (LPA) Samantha Salunga conducted an announced Pre-Licensing (follow-up) virtual inspection with Josef Fradkin and Devorah Fradkin (Applicant). Due to COVID-19 state of emergency, this inspection was completed via video conferencing (Zoom). Initial pre-licensing inspection was conducted on 12/20/2020, when LPA conducted an on site inspection to measure and inspect all indoor activity space. LPA observed the outdoor play space to be located in various areas within the campus, however, did not have a separate fence that surrounds each area, so no measurements were taken of the outdoor play space. After further review, it is confirmed that the facility is exempt from square footage of outdoor play space. LPA discussed with Mr. and Mrs. Fradkin that the facility shall implement a detailed supervision plan during outdoor play time to ensure that appropriate supervision is being provided to keep the children safe and away from hazardous areas within the campus (i.e. parking lot, construction area, and various tress/bushes behind the basketball court). Mr. and Mrs. Fradkin stated that they will create a plan and submit it to LPA to place in their file no later than 01/18/2021.
Facility is in compliance with Title 22 therefore, a license for 300 school age children (age 5-14 years old) is granted and effective today's date 01/14/2021.
Exit interview was conducted with Mr. & Mrs. Fradkin. A copy of this report was reviewed during inspection and will be e-mailed to Mr. & Mrs. Fradkin. They were advised that acknowledgement of the receipt of the report is to be received within twenty-four hours. This report shall be kept available at the facility for public review for 3 years. |
