On 3/21/22 at 9:10 AM, Licensing Program Analyst (LPA) Keturah Lane visited the facility to conduct an annual inspection. Upon arrival LPA met with Director Naomi Shakhman and proceeded to tour the facility. During today's inspection, there were 69 children with 13 staff in 6 classrooms. Appropriate ratios and capacity were observed. Appropriate care & visual supervision were also observed during the inspection while children were participating in circle time and activities with the teachers.

Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation. Floors appear to be clean and safe. Drinking water is readily accessible. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Children bring snacks and lunches from home. Cleaning supplies are kept separate from food and are inaccessible to children. Storage containers for solid waste have tight-fitting covers and are kept in good repair. Director stated there are no firearms or other weapons on the premises. All foods/beverages capable of rapid spoiling are stored in covered containers at 45 F or less. The facility appears to be free of insects and rodents. There is an operational carbon monoxide detector at the facility.

Outdoor play area is a fenced playground with sufficient material for cushioning. There are no bodies of water or weapons at this facility. Climbing structures, swings and slides are securely fixed to the ground. Area has trees and canopies used for shade. Equipment is age appropriate. Area has drinking water readily accessible and grounds are free of debris or potential hazards.

LPA reviewed medication storage. Director stated that medication (except for Epi-pens) are stored in a locked cabinet in the office. Epi-pens are kept in classroom in a latched cabinet. Personnel records contain health screening documentation and at least one staff member has current CPR and First Aid certifications. All staff have completed mandated reporter training and required immunizations. Each personnel record contains documentation of educational background and training. Sign ins were reviewed. Children’s records contain admission agreements and medical assessment. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA Lane reviewed Covid-19 procedures and protocols with Director Naomi Shakhman. Director stated that they have a covid-19 task force and pediatrician that they consult with. Facility is still following a travel policy that if a family travels outside of California they must quarantine for 7 days before returning to the program. Director states they are following the decision tree, requiring all children to be tested (not just those with symptoms) in the classroom that reports a positive case immediately and again within 5 days. No visitors are allowed in the building, including parents. All drop-offs and pick-ups are done at the front entrance. Staff are called to escort children to the front door. Director stated children are in cohorts and teachers stay within the same cohorts. (continued on LIC809-C)...

Celebrations and special events are done outside and cohorts are staggered. Director stated that the facility required ALL children to be tested before returning from the winter break. Most of the positive cases happened over the break and the surge was because of the testing requirement and most children that tested positive were asymptomatic. Facility has been following all sanitization and disinfection protocols. All staff are wearing masks inside the facility. Director stated that prior to March 12, 2022 a majority of the children (over 2 years of age) were wearing masks inside the classrooms.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
