On December 15, 2022 at 1:00 pm, Licensing Program Analysts (LPA), Jessica Rubio and Ana Noble conducted an annual inspection as part of a compliance review. LPA met with Director Kristen Erwin. A tour of the inside and outside of the facility was granted and the following was observed and/or noted:
· The following items were posted and updated where necessary:
- License
- Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148)
- Parent’s Rights Poster (PUB393)
- Personal Rights (LIC613A)
- Child Car Seat Law
- Menu
· The facility is operating within the limits as stated on the license. LPAs observed 8 children in care with Director and an assistant providing care and supervision.
· Ratios are being met during this inspection
· Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards
· There are no weapons present at the facility as stated by Director Kristen Erwin.
· There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Drinking water is provided in the indoor and outdoor activity space by filtered water supply
· There are no medications stored in the facility at this time as stated by Director.
· Hazardous items were not stored where inaccessible to children. LPA observed an accessible drawer in teh kitchen area containing screwdrivers and pliers. A citation will be issued.
· Poisons and toxins are locked and inaccessible to children
· All floors were observed to be clean and safe

· Bathrooms were observed to be safe, sanitary and in operating condition
· Playgrounds are not enclosed by appropriate fences and free of hazards. LPAs observed the perimeter of the facility outdoor area to not be completely fenced leading into an apartment complex. A playground area is also located adjacent to that unfenced area. A citation will be issued.
· Outdoor activity areas are supplied with age and size appropriate equipment however not in good condition. LPA Noble observed broken fencing in the picnic area, broken toys and playground equipment in the picnic and playground area. LPAs also observed a semi trailer adjacent to the blacktop and playground area which is accessible to children. A citation will be issued.
· Food preparation area is clean, free of litter and rubbish and free of rodents and other vermin
· Food is stored appropriately and protected from contamination
· All storage containers for solid waste were observed to have tight-fitting covers that are kept on, and in good repair
· Sign in/Sign out record were reviewed and meets regulation requirements
· Disaster drills are conducted at least every six months – last drill conducted on 11/18/2022
A review of two staff and five children's records were conducted as part of this evaluation.
· Children’s records were found to be incomplete during this inspection. One child (Ref#4) was missing a medical assessment and TB test/risk assessment. A citation will be issued.
· Director and staff (Ref#2) were missing or had expired mandated reporter training. A citation will be issued.
· Director's First Aid/CPR is expired and staff (Ref#2) did not have a current First Aid/CPR card. A citation will be issued.
· Staff records review indicates that all staff present meet minimum qualifications for the position for which they were hired.
· A staff member was not present with current Pediatric CPR/First Aid….
· Director completed Health and Safety Training in Spring 1980.
· A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following items were discussed with the Licensee/Director during inspection:


· Menus shall be posted at least one week in advance in a place visible by the child’s authorized representative, dated and kept on file for 30 days, and made available upon request.
· The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall
· The Licensee was informed of their reporting requirements and provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO10@dss.ca.gov
· The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations858@dss.ca.gov

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Direotor was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
