32 | A review of staff records on this date indicates that two staff members (staff members #1 & #2, see LIC811 page) didn’t have a criminal record clearance. Director provided documentation the live scan was completed for staff members #1 & #2, based on review of the LIC9163 - Request for Live Scan. However, the incorrect facility number was listed on the form. Staff members #1 and #2 have been employed with the facility for several months. Capacity and limitations as specified on the license are being maintained, per observation. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. The name of the childcare center director or fully qualified teacher(s) designated to act in the director’s absence has been reported to the Department, per file review. Sign in/out sheets reviewed and observed to have the parent’s/authorized representative’s full legal signatures and record the time of day. Children were observed to be under supervision, including visual supervision. Based on observation, the facility maintains a ratio of one teacher supervising no more than 12 children in care. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening, and current documentation of completed mandated reporter training. Based on staff file review, two staff members were missing the required immunizations. Menus are posted at least one week in advance where an authorized representative can view them.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
This facility does not provide Incidental Medical Services (IMS). LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services.
