Licensing Program Analyst (LPA) Jennifer Lott conducted an unannounced Case Management visit. LPA was greeted at the front door by Director, Monica Collins and was granted entry after identifying herself and disclosing the purpose of her visit. LPA toured the facility. The facility was within their licensed ratios/capacity.

The visit was initiated due to a recent change in Director. LPA reviewed the requirements for a new director and provided required paperwork.

Based on today’s visit, no deficiencies were observed at this time. An exit interview conducted and report was reviewed with Director, Collins. A notice of site visit was given and must remain posted for 30 days.
