25 | On 9/8/23 at 10:20 AM, Licensing Program Analysts (LPA) Keturah Lane and Gerald Poindexter conducted an unannounced case management inspection at request of Licensee. Upon arrival, LPAs met with Assistant Director Katrina Hoffman. LPAs observed a total of 26 infants in care:
Room 9 (mobile) – 8 infants with staff members Adriana Anzaldo and Victoria Villaneuva
Room 11 – 11 infants with staff members Raelissa Taylor, Karolinn Cone-Luna and Eugenia Navas
Room 1 (temporary room) – 7 infants with staff members Lydia Longworth and Karolina Dos Santos
An emergency request was made back on 7/17/23 to allow 8 children from the infant license to utilize a room licensed for the preschool (#376600321) due to a leak in the ceiling and repairs needed in classroom 10. The fire clearance was granted on 7/26/23, the emergency request was granted on 8/1/23 and facility was allowed to use classroom #1 expiring upon completion of repair or 9/15/23.
On 9/1/23, LPA Lane received an e-mail from the facility that repairs in classroom 10 have been completed and the facility is ready for LPA to approve the classroom for use. LPA inspected classroom 10 for repairs on 9/8/23 and did not observe any immediate or potential hazards. LPA will update the license to remove classroom 1 from the infant license effective Monday 9/11/23 and add it back to the preschool license (#376600321).
A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Appropriate ratio and capacity were observed. No deficiencies were cited at this inspection. Exit interview conducted and report was reviewed with facility representative, Assistant Director Katrina Hoffman. Notice of site visit was provided and must remain posted for 30 days.
