On 7/19/23 at 8:00 AM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced case management inspection at request of Licensee. An emergency request was made due to a leak in classroom 10 and part of the ceiling fell down and needs to be repaired. Classroom 10 was closed immediately and Director Daphne Landa reported the incident to licensing via duty line and written report on Monday 7/17/23. No children or staff were injured. Facility wishes to use Classroom 1 under the preschool license (#376600321) temporarily for the toddlers displaced from classroom 10. Fire clearance was ordered prior to today's visit.

Upon arrival, LPA met with program specialist Lauren Greenfield and toured the facility. LPA observed a total of 18 infants in care:
  • Room 9 (mobile) - 9 infants with staff members Victoria Villaneuva, Barbara Marenick and Adriana Anzaldo
  • Room 11 - 9 infants with staff members Raelissa Taylor, Charlotte Conroy and Erika Bustos
  • Room 10 closed for maintenance.


A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. The licensee has not exceeded the conditions, limitations and capacity specified on the license. LPA inspected classrooms and did not observe any immediate or potential hazards. LPA observed appropriate ratios and supervision.

LPA inspected classroom 1 to be used temporarily until classroom 10 is repaired. The room has an outdoor and indoor exit, 2 toilets, 3 sinks for children and additional sink on counter. There are two shelves (one is empty) with toys and 1 table with 8 toddler sized chairs. There are cots stacked in the room and a costume area with hooks. Cubbies are on one side of the wall. The rest of the room is empty. Room has adequate A/C, lighting and ventilation. Bathrooms are operational but soap and paper towels need to be added. LPA did not observe a separate sleep area, nor is there a changing table.

Director Daphne Landa was not available at the facility. LPA spoke with Director Daphne Landa on the phone around 9:15 AM regarding plans for the room and additional questions. Assistant Director Katrina Hoffman arrived around 9:30 AM. Director Landa stated they will install a changing table arms length from the sink, move additional furnishing and toys into the classroom, clean the floors, wipe down the walls and add soap/paper towels before infants use the classroom. The room will only be used by mobile infants 12 months or older that can use cots. LPA reminded Director to use age-appropriate toys and equipment in the classroom with infants and to send pictures once the additions are made to the classroom. Once fire clearance has been granted and photos of the additional requirements have been sent to Licensing, classroom 10 may be temporarily used for the infant license until room 10 has been repaired. Classroom 10 is the same dimensions as Classroom 1 so there will be no changes to the overall capacity of the license which is 32 infants. Upon approval, an updated temporary license will be mailed to the facility. Facility expects repairs to be completed by early August.

No deficiencies were cited at this inspection.

Exit interview conducted and report was reviewed with facility representative Lauren Greenfield. A notice of site visit was provided and must remain posted for 30 days.
