13 | At 9:10AM on September 26, 2022, Licensing Program Analyst (LPA) Alaina Wilburn conducted an unannounced complaint visit. LPA met with Assistant Director Wilma Cabrera, to deliver findings on the above stated allegation.
Investigation consisted of; interviews with Staff, Children and potential witness.
On 08/11/2022, a complaint allegation was received by the Community Care Licensing (CCL) office that Staff did not observe day care child in soiled clothing. During interviews, LPA Wilburn observed two separate incidents of questionable supervision involving Child #1 (C1). In the first incident, C1s parent arrived for pick up and observed C1 in a different outfit. It was determined that C1 had an accident and soiled themselves. C1 went to the cubbie, grabbed a bag, went to the restroom, changed out of the soiled clothing and returned the bag with the soiled clothing to the cubbie without the staffs awareness or knowledge of the incident. In the second incident, C1 was playing outside with friends. It’s alleged that C2 shoved rubber wood chips from the playground in C1’s underwear. C2 denies incident and staff interviewed denied observing such incident; however, later that evening at home, while C1's parent was preparing child to be bathed, parent observed |
