A required – 1 year inspection is being conducted as part of a compliance review. Licensing Program Analyst (LPA), Otsanya Cameron conducted a COVID-19 Facility Prescreening prior to inspection. LPA toured the infant center, inside and out. The following was observed: this is a combination center in which two other programs ( school age 376600112 and preschool 376600088) were also inspected on this date
· A review of the staff records and a sampling of the children's records were conducted as part of this evaluation. See Confidential Names List (LIC811)
· The licensee is asked to update the following documents, if applicable, and submit to licensing within 30 days:
1. LIC 500 Personnel Report
2. LIC 610 Emergency & Disaster Plan
3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made)

· The facility is operating within the terms of the license
· Ratios were met during this inspection
· Appropriate supervision was provided during this inspection
· All activity areas are physically separated from other components
· Drinking water is provided by Individual Drinking cups and Water pitcher outside
· Rooms are equipped with age appropriate furniture and equipment in good condition
· Napping equipment is sufficient for the capacity and meets licensing requirements.
· No weapons stored at the facility
· There are no bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Medications are stored where inaccessible to infants
· Hazards are stored where inaccessible to infants
· Toxins are locked
· Toileting area was observed to be safe, sanitary and in operating condition
· Infant changing tables contain a padded surface no less than one inch thick and be covered with washable vinyl or plastic; and contain raised sides at least three inches high
· Outdoor play area is physically separated by appropriate fencing and free of hazards
· Outdoor activity areas are supplied with age and size appropriate equipment in good condition
· Food preparation area is clean and free of vermin
· Food is stored appropriately and protected from contamination
· Individual feeding plans were reviewed
· Infant needs and services plans were reviewed
· Individual Sleeping plans LIC9227 were not on file for infants as all children in care are over the age of 12months.
· Sign in/Sign out record was reviewed and meets regulation requirements
· A Staff member is present with current Pediatric CPR/First Aid which expires on 3/2022

· Opening and closing staff member’s CPR/First Aid expires on 3/2022
· Director completed Health and Safety Training in the fall of 1995/ Lead and Nutrition component has not yet been completed
