On November 3, 2022, Licensing Program Analyst (LPA), Jessica Rubio arrived at the facility to conduct an inspection for adding a Toddler component to the license. The facility requested a capacity of 16 children for the toddler option. LPA met with Director Norma Terrazas. LPA measured the indoor toddler space (Room #5) as well as the designated outdoor toddler area.

Measurements were taken and the following was determined:
Indoor Activity Areas – Room #5.
LPA has determined that there is sufficient space to accommodate 12 children.
There is one toilet and two sinks for children’s use, as well as a changing table and sink for staff use.

Outdoor Activity Area - Toddler Playground Area
LPA has determined that there is sufficient space to accommodate 12 children.

Limiting factor for the Toddler program capacity is indoor activity area of 12 children.

On November 3, 2022, LPA Jessica Rubio also conducted an annual inspection as part of a compliance review. A tour of the inside and outside of the facility was granted and the following was observed and/or noted:

· The facility is operating within the limits as stated on the license.
· Ratios are being met during this inspection
· Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards
· There are no weapons present at the facility as stated by Director Norma Terrazas.
· There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Drinking water is provided in the indoor and in the outdoor activity space by filtered water supply
· Medications are stored where inaccessible to children on the top of the refrigerator in the staff lounge area.
· Hazardous items are stored where inaccessible to children in classrooms, however LPA observed cleaning solutions in an unlocked kitchen sink cabinet in the staff lounge. Children were not observed in the area, however there was no working barrier/safety gate or door preventing access to the lounge area. A citation will be issued.
· All floors were observed to be clean and safe
· Bathrooms were observed to be safe, sanitary and in operating condition. Classroom 4 had two sinks that were not working however there was a third operable sink available for children’s use. Director stated sensors for the inoperable sinks have already been ordered.
· Playgrounds are enclosed by appropriate fences and free of hazards
· Outdoor activity areas are supplied with age and size appropriate equipment in good condition
· Food preparation area is clean, free of litter and rubbish and free of rodents and other vermin
· Food is stored appropriately and protected from contamination
· All storage containers for solid waste were observed to have tight-fitting covers that are kept on, and in good repair
· Sign in/Sign out record was reviewed and meets regulation requirements
· Disaster drills are conducted at least every six months – last drill conducted on 9/22/2022
A review of six staff and five children's records were conducted as part of this evaluation.
· All children’s records were found to be complete during this inspection.
· All staff records were found to be complete during this inspection.
· Staff records review indicates that all staff present meet minimum qualifications for the position for which they were hired.

· A staff member is present with current Pediatric CPR/First Aid which expires on 2/2024

· A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following items were discussed with the Director during inspection:


· Menus shall be posted at least one week in advance in a place visible by the child’s authorized representative, dated and kept on file for 30 days, and made available upon request.
· The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall
· The Licensee was informed of their reporting requirements and provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO10@dss.ca.gov
· The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations858@dss.ca.gov

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The facility is being cited for Title 22 Regulation Section 101238 (g). See LIC809-D for cited deficiencies.
