Licensing Program Analysts (LPAs) Sumayya Habeebulla and James Wilkerson arrived at the facility to conclude the investigation into the above allegations. An initial visit was conducted on 09/27/21 and extended at that time. LPAs toured the facility and conducted census. During this investigation one staff and the Director were interviewed. During the entire investigation 4 children were interviewed.
There was an allegation that children are not required to wear masks. LPAs observed during the initial visit that one child was wearing a mask and all the other children were not since they were in a playground. The director was present at that time, and she was wearing a mask. LPAs received conflicting information on whether children are being encouraged to wear face coverings or not. LPAs are unable to prove that this allegation is true or not. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the allegation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

The second allegation was that Staff wear masks except when a parent is present who doesn't believe in masks. The staff were observed to be wearing masks; however LPAs are unable to prove that the staff were not wearing masks in front of parents who don’t believe in masks. During LPAs visits the staff that were present were wearing masks. LPAs are unable to prove that this allegation is true or not. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the allegation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

The third allegation was that the Staff was not tested for COVID after having symptoms. From the information received from the interviews of staff, the facility requires staff to get tested if they show symptoms of COVID. LPAs are unable to prove that this allegation is true or not. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the allegation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

The fourth allegation was that children are allowed to stay at the day-care with cold symptoms. From the information received from interviews of staff, the children are not kept in classrooms if they show signs if illness. LPAs are unable to prove that this allegation is true or not. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the allegation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

An exit interview was conducted, and a Notice of Site Visit was posted on this date.
