25 | On 3/29/23 at 3:36pm, Licensing Program Analyst (LPA) Saraliz Velando, conducted an unannounced Case Management Inspection for reported Lead Exceedance. LPA met with Director, Leone Power, and toured the facility. Aso present in the facility were 64 daycare children and 10 teachers/staff. Facility was within ratio & capacity. Staff members have the required Criminal record clearance and are associated to the facility. LPA interviewed staff, examined the faucets, and inspected the fixtures deemed an Action Level Exceedance.
Fixture reported with 5.5 ppb or greater lead exceedance levels was as follows:
• “C” Drinking Fountain located in PreK-B Room – 15.2 ppb
• “I” Drinking Fountain located in 2’s Room– 15.4 ppb
• “Q” Drinking Fountain located in 3’s Room – 8.55 ppb
• “T” Drinking Fountain located in Preschool outdoor play area – 6.79 ppb
Licensee’s testing was completed on 12/30/22. Licensee reported that drinking fountain "C" was removed on 3/11/23. Drinking fountain “I” was removed on 3/11/23. Drinking fountain “Q” was repaired on 1/25/23. Drinking fountain “T” was repaired on 1/25/23. Drinking fountains “C”, “Q”, and “T” have been retested on 3/25/23. Facilities Manager advised the director not to shut off valves and wait for retesting results. The retested fixtures have been covered and taped off to cease use by children or staff.

Exit interview was conducted and report was reviewed with the Director, Leone Power. A copy of Appeal Rights and Notice of Site Visit was given and must remain posted for 30 days. |
