13 | On October 15, 2020, at 10:30am, Licensing Program Analyst (LPA), Martha Malane conducted an unannounced video-conference to deliver findings regarding the above complaint allegation via FaceTime, due to the COVID-19 outbreak, and met with the director and assistant director.
It was alleged that the facility is operating out of ratio. During the course of the investigation, LPA conducted interviews with the director, the assistant director, four (4) staff members and several day care parents which resulted in conflicting statements. Staff time sheets, child sign-in/out sheets and daily classroom attendance records were reviewed. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
A copy of the complaint investigation report LIC9102 Technical Advisory and appeal rights (LIC 9058) will be e-mailed to the director and the director was advised that acknowledgement of receipt of the report and appeal rights are to be received within twenty-four hours.
No deficiencies cited. An exit interview was conducted
