13 | On 06/17/22 at 1:30pm, Licensing Program Analyst (LPA) Adrian Castellon conducted an unannounced inspection to deliver complaint findings for the above allegation. LPA Castellon met with director Ana King and discussed the purpose of the inspection. It was alleged that a day-care chid was injured while in care. During the course of the investigation, three unannounced inspections were conducted. Interviews were conducted with day-care parents and facility staff. Documents (diaper change daily reports, feeding daily reports, texts, incident reports, emails, texts, and roster) were obtained pertaining to the allegations.
Based on the staff admission and interviews conducted, the allegation is substantiated. Child in care was injured due to a lack of supervision. Child fell out a feeding chair while being fed. As such, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Violations of the California Code of Regulations, Tittle 22, are being cited on the attached LIC9099D. Final findings delivered as substantiated, a type ‘A’ violation is being issued, as the situation is an immediate risk to children in care. Appeal rights discussed and given on this date. LIC 9224 requirement was discussed on this date. LIC9224 was given to director King. |
