On May 12, 2020 at 3:00 p.m. Licensing Program Analyst Leilani Curtis conducted an unannounced inspection via Zoom for the purpose of delivering the findings for the above allegation. It was alleged that the Licensee is not adhering to the payment provision of the admission agreement. The initial tele-visit was conducted by LPA Curtis on 04/06/2020. During today's inspection LPA Curtis met with Director, Marina Sragovicz and toured the facility. Also present were 14 children (in two groups) outside on the playground with staff Paul Torres and Daisy Perez. In the course of this investigation LPA interviewed the Director and Complainant. The Admission Agreement, Parent Handbook and several parent emails were obtained and reviewed.
Based on interviews conducted and documents reviewed the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, Title 22, 101219(f) is being cited on the attached LIC 9099D. A copy of this report as well as a copy of the appeal rights were emailed to the Director at the conclusion of the interview. The Director will confirm receipt of this report via e-mail and the reply of confirmation will serve as the signature acknowledging these rights. The Director will post the Notice of Site Visit and it must remain posted for 30 days. |
