On 07/20/2021 at 02:00 PM LPA Dana Stevens conducted an unannounced visit for the purpose of delivering investgative findings for the above allegations. During the course of the investigation, all parties relevant to the complaint allegations were interviewed, documentation was reviewed and facility and equipment were inspected.

Steps are not age appropriate
All staff interviewed stated that the children are always assisted with using steps and staff stand behind the children to ensure they do not fall, however manufacturer age recommendations for the steps were not available. Based on LPA's observation and staff interviews the steps do not present a hazard to children when staff are assisting children, however due to the absence of manufacturer age recommendations this allegation is deemed Unsubstantiated.

Children are force fed.
The department received information that children are forced to eat at meal times, however all staff interviewed stated that children are never forced to continue eating once they appear to be full. Parents interviewed expressed confidence that children are fed appropriate amounts. Based on conflicting information this allegation is deemed Unsubstantiated.

Child was grabbed inappropriately by the arm.
The department received information that Licensee was observed grabbing a child inappropriately by the arm, however all staff interviewed stated that neither Licensee nor staff grab children forcefully. Licensee and one staff stated that sometimes they will hold children by the arm when assisting them with steps or to get the child's attention when directing them in activities. Based on conflicting information this allegation is deemed Unsubstantiated.


Child was placed in the Timeout chair for an extended amount of time
The department received information that children are left in the Timeout chair for extended amounts of time. Staff interviewed stated that children are usually placed in the Timeout chair for 2-3 minutes, however Licensee stated she does not set a timer when children are placed in the TImeout chair. Based on a lack of preponderance of evidence this allegation is deemed Unsubstantiated.

A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.

No deficiencies cited.

An exit interview was conducted and a copy of this report, appeal rights and Notice of Site Visit were provided to Licensee.

Notice of Site Visit must be posted for 30 days.
