25 | On 05/20/2022 at 12:00 PM, Licensing Program Analyst (LPA), Dana Stevens conducted an unannounced Annual Required Inspection and met with licensee, Eunice Vasquez. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Licensee. There were 2 daycare children present at the time of the inspection, ages 11 months, and 2 yrs. Licensee accompanied LPA throughout the inspection of this 2 bedroom, 1 bathroom home. The following areas are used for child care: Daycare room, bedroom 1 and bathroom. Off-limits areas are kitchen, upstairs bedroom, backyard and detached garage. These areas are made inaccessible with door locks.
The fire extinguisher, smoke detector, and carbon monoxide detector met requirements. The licensee has toys, play equipment and materials available. Licensee stated there are no weapons in the home. A review of staff records on this date indicates that all individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Licensee’s First Aid and CPR certifications expired 03/2021, and Licensees are scheduled to renew CPR in June 2022. Licensee has required immunization. Facility roster was not available.
There is one crib or play yard for each infant who is unable to climb out of the crib or play yard. LPA observed blankets and pillows in the play yard while infant was sleeping. The provider physically checks on sleeping infants every 15 minutes however licensee did not have documentation of the sleep checks. An Individual Infant Sleeping Plan (LIC 9227) was not maintained for the one infant present less than 12 months of age. The provider places infants up to 12 months of age on their backs for sleeping. LPA observed 3 prohibited items present in the daycare room during this inspection, a baby bouncer, an excersaucer, and a baby walker. |
