25 | Licensing Program Analyst (LPA), Tyra Block, conducted an unannounced annual inspection with the Licensee, Olga Levenzon accompanied by her mother Rimma. Present at the home was 1 daycare child and licensee's son. The single story 3 bedroom home was toured and inspected to ensure an environment safe for the care and supervision of children. The fire extinguisher (3A40BC), smoke detector, and carbon monoxide detector meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. There are no bodies of water on the property. Licensee states that there are no weapons in the home. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions, except for licensee's adult son. First Aid/ CPR certifications expired 6/2019 and Mandated Reporter training was completed 10/31/20. Licensee has required immunizations. Children’s records were reviewed and deficiencies discussed with licensee. Notification of Parent's Rights and immunization record was not on file for child currently enrolled. No drills have been conducted or documented. The fireplace was barricaded and made inaccessible.
Licensee has provided adequate space for the children to eat, sleep and play within the home. Areas used for child care include Child's room, hall bathroom, and first bedroom on the left. Off limits areas include all other bedrooms, living room, kitchen, and garage. They are inaccessible through use of child safety gate and door locks. The licensee has sufficient toys and equipment available for children and a fenced backyard available for outdoor activities.
Provider is hereby reminded of the following: Advised no changes should be made to the home without prior notice and/or approval from Licensing, including use of an area designated as off-limits; report suspected child abuse and neglect, maintain children’s records according to regulation, post all required forms, COVID guidance; corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers are not allowed in day care. All equipment that is used should be used only as intended by the manufacturer. Licensee was also provided with information regarding Safe Sleep Regulations/SIDS and Shaken Baby Syndrome.
