An unannounced random inspection was conducted today by LPA, Nancy Diaz. Upon arrival LPA observed 3 children in care with the licensee. Helper Svetlana Andriyenko arrived at 8:30. Four more children arrived during the inspection, making the total of 7 children. Children were participating in various indoor activities. LPA conducted a tour of the home to ensure the health and safety of children. Licensee is using the following areas for daycare: family room, back fenced yard, two bedrooms for napping and the hallway bathroom. The following areas are off-limits to children: living room, kitchen, dining and the second floor of the home. These areas are inaccessible to children via barricades.

There is a pool present in the yard that is accessible due to a broken fence. Mrs. Galina stated that she does not maintain any weapons in the home. All cleaners, toxics and other hazardous substances are inaccessible to children in care via barricade between the family room and kitchen area. Fireplace is screened to prevent access by children. Fire extinguisher and smoke detectors present in the home meet State Fire Marshall standards. A carbon monoxide is also maintained in the home. The home is kept clean and orderly with sufficient ventilation for safety and comfort. The home provides toys, play equipment and materials. The home maintains a working telephone service.

Outdoor play areas are fenced. The licensee maintains current emergency information on all the children. The licensee and other personnel has completed training on Preventative Health Practices including Pediatric CPR and First aid. Licensee’s and helper's CPR & First Aid certificate are valid through June 23, 2020.

A handout was provided to the licensee today on “Effects of Lead Exposure”. Licensee shall provide a copy of this handout to all the daycare parents.

All licensees shall ensure the inaccessibility of pools (in-ground and above-ground), fixed-in-place wading pools, hot tubs, spas, fish ponds and similar bodies of water through a pool cover or by surrounding the pool with a fence.

This regulation requirement was not met as evidenced by LPA's observation. There is a pool in the backyard that is appropriately fenced, however; part of the fence material has broken off, leaving a gap that that is approximately 2 feet wide. A child can easily slip through the gap. This poses a direct and immediate risk to the health, safety or personal rights of children in care.

Facility has not exceeded the capacity specified on the license. Licensee resides in this home with her son, Michail Stepanenko and mother Tubba Stepanenko. There are no new adults living or working in the home over the age of 18 years. All individuals subject to criminal record review have obtained criminal record clearance or exemption prior to working, residing or volunteering.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee and helper are exempt from Mandated Reporter AB1207 training certification due to applicant having limited English proficiency, Licensee's and helper's primary language is Russian.



TYPE A DEFICIENCY WAS CITED TODAY. CIVIL PENALTY WAS ALSO ASSESSED. Type A violation if not corrected, will have a direct and immediate risk to the health, safety, or personal rights of children in care.

Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.

LPA observed the Representative post the Notice of Site Visit in a prominent place. The Representative states it is understood that this notice must be posted for 30 days.
