25 | On 3/30/23 at 9:00am Licensing Program Analyst (LPA) Patrick Ma conducted an unannounced annual inspection. Upon arrival, LPA met with Licensee Desarae Lewis. Also, in the home was helper/husband Rick Lewis, adult son, Clayton Lewis, and friend Miranda Lukens. Licensee was provided the Inspection Checklist (LIC 126). The 2 story home was toured and inspected to ensure an environment safe for the care and supervision of children. Present in the home were 6 day care children.
Licensee has provided adequate space for the children to eat, sleep and play within the home. Areas used for child care include living room, dining room, kitchen, play room, game room, bathroom 3, and back yard. During inspection, License requested to remove upstairs areas bedroom 2, bedroom 3, and bedroom 4 from license making the entire upstairs off-limit and made inaccessible with a gate at the bottom of the stairs. Off limits areas also include, right and left side of back yard and garage and are inaccessible through use of door locks and gates. The licensee has sufficient toys and equipment available. The home has a fenced backyard available for outdoor activities. There is a pool in the back yard which is properly fenced and made inaccessible with a self-latching gate that swings away from the pool. Licensee understands that supervision is required at all times during outdoor activities. LPA conducted child care quality management interview with the Licensee.
The fire extinguisher, smoke detector, and carbon monoxide detector meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. Licensee states that there are no weapons in the home. First Aid and CPR certifications expire on 5/2023. Licensee has required immunizations. Licensee completed Mandated Reporter Training on 11/14/22 and is reminded it must be completed every 2 years. Children’s and Staff records were reviewed and found to be in order. |
