On 8/15/2022, at 9:05am, Licensing Program Analyst (LPA), Cindy Meier and conducted an unannounced Annual Inspection and met with Co-Licensees, Elaine Wong and Jeane Wong. LPA disclosed the purpose of the inspection and was led on a tour of the facility indoors and outdoors. This facility is a two story, two-bedroom, two-bathroom and office house. The main daycare area is a multi-leveled, fenced outdoor area with shade coverings. The entrance to the daycare area is through a side gate. Children and parents do not enter the main home for drop off or pick up. The areas used upstairs is the bathroom and library. Off limits areas include the entire main house. Children do not have access to the main house. Stairs are barricaded by gates.
Hours of operation hours are Monday – Friday 7:30am to 4:00pm. The 3rd Wednesday of each month is designated as a field trip day. There were thirteen (13) children present during the inspection, four (4) preschoolers, nine (9) school age children.

The fire extinguisher, smoke detector and carbon monoxide detector met requirements. Hazardous items were inaccessible to children in care. LPA informed licensee poisons shall be placed in a storage area and locked. LPA did not observe any poisons during the inspection. The storage area for poisons is locked. LPA observed toys and materials available for children’s use. LPA informed licensee to ensure children are supervised at all times during outdoor activities. Licensee stated there are no bodies of water and LPA did not observe any bodies of water during the inspection. Licensee stated there are no firearms, other weapons or ammunition in the home.

A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions.
A review of staff records on this date indicates that co-licensee’s and two (2) assistants TB or immunizations were not available for review. See 809-D.
LPA reviewed children’s files. Children’s files did not have available for review the LIC282 Liability Insurance form. See 809-D.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Co-licensees or two (2) assistants have not completed the Mandated Reporter AB1207 training. Pediatric CPR and First Aid certifications expire on 08/2023. Facility roster is maintained and was reviewed. Co-licensee does not have record of documented fire drills. Required documents are posted.

Incidental Medical Services (IMS) policy were discussed. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA discussed safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Per California Code of Regulations, (Title 22, division 12 & Chapter 3) three (3) Type B citations and one (1) Technical Advisory are being cited on the attached LIC 809-D.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted and report reviewed with Co-Licensee, Elaine Wong. Licensee was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights.
