13 | Licensing Program Analysts (LPAs) Selina Siao and Elizabeth Rivera conducted unannounced inspection to deliver the above complaint finding. The initial inspection was conducted on 04/11/2019. Upon arrival, there were 7 children including 2 school age children at the facility while licensee's adult grand daughter were taking 3 of her cousins to their house.
Throughout the course of investigation, records were reviewed, and interviews were conducted with licensee, helper, several daycare parents and several day care children. Although Licensee denied that her son in law Terrance Johnson lived at the facility but the 30-day notice to terminate tenancy dated 03/18/2019 obtained from licensee listed him as a tenant. Based on information obtained and interviews conducted, and record review the preponderance of evidence standard has been met, therefore the above allegation regarding uncleared adult living in the home is found to be SUBSTANTIATED, California Code of Regulations, 102370(d) is being cited on the attached LIC 9099D. Civil penalty of $500 is being assessed today. This report was translated to licensee by LPA Elizabeth Rivera in Spanish.
“Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. “
