13 | On 01/23/2023 at 10:30 a.m., Licensing program Analyst (LPA) Rajani Goudreau conducted an unannounced complaint inspection for the purpose of delivering the complaint findings to the above listed allegations. Upon arrival LPA met with Licensee, Onika Miller, and proceeded to tour the facility. During the inspection there were six children in care; including three infants and three preschool age children. During the course of the investigation, records were obtained and reviewed from the facility, interviews were conducted with the licensee, helper, children in care and daycare parents and observations were made.
On November 14, 2022, Community Care Licensing (CCL) received a complaint alleging licensee does not properly maintain the home and the smoke detector is in disrepair. At time of interview with licensee on November 15, 2022, licensee denied the home not being properly maintained. Also, licensee denied an incident occurring resulting in a child getting hurt due to the inside or outside of the home not being properly maintained. Based on observation, LPA observed the inside and outside of the home to be clean and orderly. Licensee denied the smoke detectors in the home being in disrepair. See LIC9099-C continuation page...
