25 | On September 8th, 2021 at 1:10 PM, Licensing Program Analyst (LPA) Jo Ann Legaspi conducted an unannounced Annual Required Inspection and met with Licensee Sahra Hayder. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Licensee. Six (6) children, two (2) staff and the Licensee were present in the facility during this inspection. This facility is a one story, four (4) bedroom, two (2) bathroom house. Licensee accompanied LPA inside and out of the facility during this inspection. The following areas used for childcare are: living room, dining-room, play room, one bedroom and one bathroom. The off limits areas are the kitchen, remaining bedrooms and remaining bathroom. The kitchen is made inaccessible to children in care with safety gates; the remaining off limit rooms are made inaccessible through the use of door safety knobs.
The fire extinguisher, smoke detector, and carbon monoxide detector met requirements. Hazardous items were observed inaccessible to children during this inspection. The Licensee has available toys, play equipment and materials. The home has an available fenced backyard for backyard activities. Licensee was reminded that continuous supervision is to be given to children whenever engaged in outdoor activities. No bodies of waters were observed on the premises during the inspection. Licensee stated there are no weapons in the home. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Licensee’s First Aid and CPR certifications expire in 08/2023. Licensee has required immunizations. Licensee completed Mandated Reporter Training on 09/08/2021. The facility roster is maintained and was reviewed. The last fire and disaster drills were conducted and documented on 08/13/2021. There is one crib or play yard for each infant who is unable to climb out of the crib or play yard. Cribs or play yards are free from all loose articles and objects. The Provider physically checks on sleeping infants every fifteen (15) minutes. There are currently no children aged 12 months or younger in care.
