25 | Licensing Program Analyst (LPA), Tyra Block conducted an unannounced annual inspection with the Licensee, Sali Hammo. The two story home was toured and inspected to ensure an environment safe for the care and supervision of children. Present were the Licensee, her minor son, and no day care children. The fire extinguisher (2A10BC), smoke detector, and carbon monoxide detector meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. There are no bodies of water on the property (pool located in the complex far from home). Licensee states that there are no weapons in the home, however, LPA discussed proper storage of firearm and ammunition. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. First Aid and CPR certifications have expired, licensee was certified online but will enroll in-person along with helper. Licensee has required immunizations and is currently exempt from mandated reporter training due training not available in licensee's primary language of Arabic/ Chaldean. Children’s records were reviewed and found to be in order. Stairs to second story are barricade with child safety gate and disaster drill log is maintained but not current. Last drill was comducted 7/15/21.
Licensee has provided adequate space for the children to eat, sleep and play within the home. Areas used for child care include living room, family room, kitchen, and downstairs bathroom. Off limits areas include entire second story and is made inaccessible through use of child safety gate. The licensee has activities available for children and the patio is available for outdoor activities.
Provider is hereby reminded of the following: Advised no changes should be made to the home without prior notice and/or approval from Licensing, including use of an area designated as off-limits; report suspected child abuse and neglect, maintain children’s records according to regulation, post all required forms, provide effects of lead exposure to parents, ensure that all adults living or working in the home have criminal background clearances to avoid civil penalties associated with this requirement; corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers are not allowed in day care. All equipment that is used should be used only as intended by the manufacturer. Licensee was also provided with information regarding Safe Sleep Regulations/SIDS and Shaken Baby Syndrome.
