25 | On August 13, 2021 at 11:00 AM Licensing Program Analyst (LPA),Dana Stevens conducted an unannounced Annual Required Inspection and met with the Licensee, Miriam Magana. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Licensee. 9 children (ages 13,12, 8, 7, 5, 3, 2, 1 and 10 months), and Licensee's spouse/assistant were present in the facility during this inspection. This facility is a single story, 3 bedroom, 2 bathroom house. Licensee accompanied LPA inside and out of the facility during this inspection. The following areas used for child care are: daycare room, kitchen, dining area, living room, and hall bathroom. Off limits areas are all bedrooms, master bathroom and garage. These areas are inaccessible through use of door knob covers.
The fire extinguisher, smoke detector, and carbon monoxide detector met requirements. All hazardous items were inaccessible to children. The storage area for poisons is locked. The licensee has toys, play equipment and materials available. The home has a partially fenced backyard available for outdoor activities, Licensee understands total supervision is required during outdoor play. No bodies of water observed on the premises during the inspection. Licensee stated there are no weapons in the home. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Licensee’s and spouse's First Aid/CPR certifications expire 10/2021. Licensee and spouse have required immunization. Licensee and spouse completed Mandated Reporter Training in August 2020. Facility roster was reviewed and was found current. The last fire and disaster drills were conducted and documented on 03/24/2021. There is one crib or play yard for each infant who is unable to climb out of the crib or play yard. Cribs or play yards are free from all loose articles and objects. The provider is physically checking on sleeping infants every 15 minutes. An Individual Infant Sleeping Plan [LIC 9227 (3/20)] is maintained for each infant up to 12 months of age. The provider places infants up to 12 months of age on their backs for sleeping. |
