On 7/20/20 at 4:29 PM, Licensing Program Analyst (LPA) Keturah Lane, conducted an announced Case Management Tele-Inspection visit regarding a staff member who tested positive for COVID-19. Due to COVID-19, a tele-inspection was conducted using FaceTime to tour the facility. Director Piper Rhodes was present at the tele-inspection. Census at time of report was 0 children because the facility was closed. Director stated that she contacted SD Epidemiology and was recommended to contact the families in that classroom (Pre-K) for guidance from their pediatricians and to quarantine for 14 days. Director decided to close the entire center for 14 days out of an abundance of caution to allow all staff and families to quarantine in order to deter the spread of the virus. Director also reported the unusual incident to the duty line on 7/17/20 and left a message with the assigned analyst.

Director stated that the staff member was last at the facility on July 16th when she went home with sinus congestion. The staff member tested POSITIVE for COVID-19 on Friday, July 17th, 2020. The staff member informed the center regarding the positive test result on 7/17/20. The facility will be closed 7/20/20 – 7/31/20 and will re-open on Monday, August 3rd, 2020. The staff member was only in the Pre-K classroom with a stable group of 12 children on 7/16/20. There was a total of 10 children in this classroom on 7/16/20. Census for the preschool license on 7/16/20 was 42 children. The facility also has an infant/toddler license as well. Census for the infant/toddler license on 7/16/20 was 13 children. Director notified all the families and advised staff members of the required quarantine and facility closure. Director fully sanitized the entire center. LPA Lane will continue to follow up with Director Rhodes on the facility's situation. LPA received the written LIC624 report on 7/17/20 and a follow up LIC624 on 7/20/20. Child Roster was requested on 7/20/20.

An exit interview was conducted with the Director. Appeal Rights were discussed and provided. Facility was advised to post the Notice of Site Visit for 30 days. A copy of the report will be e-mailed to the facility and director was advised that acknowledgement of the receipt of the report is to be received within twenty-four hours.
