On 1/14/2021 @ 9AM, Licensing Program Analysts (LPAs) Nancy Diaz and Jennifer Lott conducted an unannounced case management inspection via Zoom meeting. This inspection was conducted virtually due to the current COVID-19 pandemic restrictions.
LPAs met with site director Michelle Thompson. A tour of the facility was conducted. Observed present today were 74 children in the following rooms:
Room #1 with 11 children and staff Ledya Esho
Room #3 with 8 children and staff Bianca Stanley
Room #5 with 7 children and staff Claudia Almaguer
Room #6 with 6 children and staff Courtney Hatton Nielsen
Room #7 with 12 children and staff Sasha Williams
Room #8 with 8 children and staff Bianca Corrales
Room #9 with 6 children and staff Andre'l Love-Carter
Room #10 with 7 children and staff Jennifer Leuty Dillon
Room #11 with 9 children and staff Erica Boatright Ford
A review of staff records during today's inspection indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances.

Included in today's discussion were; cohort, group size and ratios indicated on the CDSS PINS 20-22-CCP (updated on September 4, 2020), Guidance related to cohorts-updated September 4, 2020 by the California Department of Public Health, PPE supplies, facility's enhanced sanitation practices, Physical distancing guidelines; use of face coverings and screening practices. Providers must implement screening procedures for staff and children.
Type B deficiency was cited today per California Code of Regulations, Title 22, Division 12 on the attached LIC 809D. An exit interview was conducted. Director will be provided a copy of this report, their appeal rights, and Notice of Site Visit via email. Director will reply to email to confirm receipt of these documents. This will act as Director's signature for today's report. Notice of Site Visit must be posted for 30 days.

REPORTING REQUIREMENTS
Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours...(d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.

This requirement was not met as evidenced by: Site director's own admittance that a staff tested positive for COVID-19 on 1/2/21. This is an unusual incident that is a potential threat to the health of children in care.
