On 6/17/24 at 12:50 PM, Licensing Program Analysts (LPAs) Keturah Lane and Adriana Macias conducted an annual inspection at the facility. Upon arrival, LPAs met with Melissa Gould who was filling in for the Director and toured the facility. During today's inspection, there were 16 children with 6 staff in 2 classrooms:

• Classroom Bunny/Pond (newborn to 1 year) had 8 infants with staff members: Maria Perez-Zamudio (teacher), Gabby Cosico (teacher’s aid), and Sara Gamboa (CNA). Deztinee Hester arrived in the classroom a few minutes after LPAs.
• Classroom Teddy Bear/Ranch (ages 1-2 years) had 9 infant/toddlers with staff members Cristy Dubois/Wager (teacher), Vicky Bautista (CNA) and Jared Hatfield (CNA). Sandra Gutierres (teacher) was a sub in the classroom and arrived a few minutes after LPAs. Appropriate ratios and capacity were observed.

Furniture and age-appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation. Floors appear to be clean and safe. Drinking water is readily accessible. Food service area consists of a kitchen which is clean and free of hazards. Menu is posted. Adequate food is available for meals. Cleaning supplies are kept separate from food and are inaccessible to children. Storage containers for solid waste have tight-fitting covers and are kept in good repair. Storage areas for poisons are locked. Director stated there are no firearms or other weapons on the premises. All foods/beverages capable of rapid spoiling are stored in covered containers at 45 F or less. The facility appears to be free of insects and rodents. There is an operational carbon monoxide detector at the facility. Last emergency drill was conducted on 5/1/24. (continued on LIC809-C...)

(A) Staff shall place infants up to 12-months of age on their backs for sleeping. (4) Infants with an Individual Infant Sleeping Plan [LIC 9227 (3/20)] that have Section C of the form completed and signed by an authorized representative shall be placed on their back when first laid down to sleep. In the event the infant changes position, the infant may remain in the alternative position. (a) Upon staff observation that the infant is capable of rolling from their back to their stomach and stomach to their back, the provider shall fill out Section D of the Individual Infant Sleeping Plan [LIC 9227 (3/20)], notify the authorized representative, and obtain the authorized representative's signature on the individual sleeping plan no later than the next business day following observation.

Outdoor play area is a fenced playground with sufficient material for cushioning. There are no bodies of water at this facility. Climbing structures, swings and slides are securely fixed to the ground. Area has canopies used for shade. Equipment is age appropriate. Area has drinking water readily accessible, and grounds are free of debris or potential hazards.

LPA reviewed medication storage. There was no prescription medication at the facility. OTC medication was labeled and in original packaging with parent forms signed. Personnel records contain health screening documentation and at least one staff member has current CPR and First Aid certifications. All staff have completed mandated reporter training and required immunizations. Infant care teachers meet educational requirements. Sign ins were reviewed. Children’s records contain admission agreements and medical assessment. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Individual Infant Sleep Plan LIC9227 was filled out incorrectly for 5 out of 7 infants present in the classroom. LPA provided technical assistance in explaining how to properly complete the form for parents.
All infant furniture & equipment are age appropriate and in good repair. There are no walkers, bouncy seats, exersaucers or jumpers in the room. High chairs or feeding tables have broad-based legs, plastic seats in good repair, and tray locked. Hazardous materials are inaccessible to children. Bottles, dishes and containers of food brought by infant’s authorized representative are labeled with the infant’s name and current date. The changing table is within arm's reach of sink. No toilet training is being provided in either classroom, all children are in diapers and toilet training is only in the preschool licensed classrooms. Caregivers wash hands before and after each feeding and each diaper change. Toys are safe, with no sharp edges, splinters or protruding points, nor small parts that can be pulled off and swallowed. Floor mats and cots are available for infants able to climb out of cribs. Cribs or napping equipment are available. Bedding is identified and stored separately for each infant. Napping infants are under visual observation by staff. Indoor activity space is physically separate from other components. Facility maintains individual feeding plans for each infant and has up to date Infant Needs and Services Plans for each infant on file. (continued on LIC809-C...)

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
