Reporting requirements was also reviewed. All personnel have the required criminal record and child abuse index clearances. LPA reviewed Emergency Disaster Plan and last fire drill was conducted on 09/13/2021. Facility has an operating carbon monoxide detector. Facility has at least one staff member that has a valid EMSA approved CPR/FA when children are in care.

When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following items were discussed with provider: Licensee was reminded that corporal punishment, smoking are not allowed in day care. Facility representative e was provided with information about Heat Related Illness, Best practice on supervision, latest car seat poster and effects of lead exposure and reporting responsibilities were discussed.

An updated personnel report and emergency disaster plan was obtained during today.
No citation issue. An exit interview was conducted, and appeal rights were provided. A notice of site visit was provided and to be posted at the facility for 30 days. Failure to keep notice posted will result in a civil penalty of $100.00
