On 4/19/24, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced visit for the complaint received on 2/20/24 for the purpose of delivering findings on the above referenced allegation. Upon arrival, LPA was greeted by District Leader Melinda Gaskin and new Director Lori Jiannuzzi.

LPA toured the facility and observed 12 toddlers in the toddler A classroom with staff members: Aida Ereikat, Leandra Mayorga and Anamika Jain. LPA observed 5 infants in the infant room with staff members Erika Torres and Laronika Linton. All staff were fingerprint cleared and associated to the facility.

During this visit LPA interviewed new Director Lori Jiannuzzi and discussed the allegations of the complaint. Based on the information obtained during interviews with staff members and parents, and documents reviewed from the facility it is determined that the facility did not change the diaper of C1 often enough during the week of 2/12 - 2/16 (2024) with one of the days being 4.5 hours between diaper changes according to the Kindercare app that staff report on. The allegation is valid because the preponderance of evidence has been met, therefore the above allegation is found to be SUBSTANTIATED. (see LIC9099-D for Type B deficiency cited).

A retraining on diapering procedures was conducted with staff members and Interim director Maddie Howerton on 2/27/24. Director Lori Jiannuzzi, start date as incoming director on 4/1/24, stated she would review the prior training with all staff including notifying parents in the comments section regarding color, consistency and rash if applicable by May 10, 2024.

Exit interview conducted and report was reviewed with Director Lori Jiannuzzi. Notice of site visit was provided and must be posted for 30 days.

Director stated she would review the trainings with staff on an on diaper changing procedures by 5/10/24. Director will send staff sign-in sheet and agenda via e-mail to LPA Lane.

Based upon Kindercare app and RP interview, C1 was not changed often enough (4 hours between changes) which may have caused a rash and is a potential health, safety and personal rights risk to children in care.

During this visit LPA interviewed new Director Lori Jiannuzzi and discussed the allegations of the complaint. Based on the information obtained during interviews with staff members and parents, and documents reviewed from the facility it is determined that staff member S1 did notice a light rash on C1 on 2/15/24 and applied cream. She stated she did not think the rash very serious and did not notify the parent upon pickup. Reporting party stated that rash was severe. During parent interviews, four out of five parents (not including reporting party) stated that the facility is regularly changing diapers every 2-3 hours. Since statements are contradictory and rashes are not reported on an incident report per facility policy, but only notated in the comments section in the Kindercare app, the allegation may have happened or is valid, but there is not a preponderance of evidence to prove that the alleged violation occurred. Therefore the allegation is Unsubstantiated.
