On 04/25/2023 at 9:00am, Licensing Program Analysts (LPA) Selina Siao conduct an unannounced annual inspection. Upon arrival, LPA met with Assistant Director Diana Evangelista and proceeded to tour the facility together. The following ratios were observed in the infant classroom # 5 upper area near the crib had 4 infants supervised by teacher Yvonne Madla and classroom #6 had 6 infants supervised by teacher Krystal Rosario and teacher aide Delina Carreno who was at the lower level area with 4 infants. All staff members have the required background clearances and are associated to the facility.

Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation and drinking water is available for the children. Storage cubbies are readily available, and room accommodates class size. Napping equipment consists of cribs for young infants and napping cots for the older infants. All three changing tables is located next to the sink. Facility provides the children’s food and prepares the children's formula at the facility. Cleaning supplies are kept out of reach of children. Outdoor play area is a fenced playground with sand as cushioning. Area has a large shade canopy and trees used for shade. Equipment is age appropriate for the children in care. LPA reviewed sign in sheets, first aid supplies and reviewed medication policy and storage, all areas are within compliance. Isolation area is the Director's office. Personnel and client records were reviewed. There is an updated needs and service plan in all the infants files and an individual safe sleeping plan for the younger infants (12 months and under). Reporting requirements was also reviewed and all staff members completed the required mandated online child abuse training. Facility has at least one staff on site with a valid EMSA approved pediatric CPR/FA certificates. LPA reviewed Emergency Disaster Plan and last fire drill was conducted on 09/2022 which is more than six months ago. Facility has an operating carbon monoxide detector located in the kitchen.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Licensee representative was reminded that corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers are not allowed in day care. The ABC’S of Safe Sleep: Sleep is Safest: Alone, on their Back in an empty Crib on a firm mattress.

Child Care Providers can now sign up for Quarterly Updates and PINS through the DSS website at https://cdss.ca.gov/inforesources/community-care-licensing/subscribe. LPA discussed California Megan's Law with provider and advised her to go on the website at www.meganslaw.ca.gov.

To improve the quality and value of the new inspection process, a survey will be sent to the email address provided. Please complete the survey and share your inspection experience. Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

All unusual incident reports shall be submitted to Licensing office via email at SDIncidentReports@dss.ca.gov or via fax at (619)767-2203.

A notice of site visit was provided and to be posted at the facility for 30 days. Failure to keep notice posted will result in a civil penalty of $100.00.
