1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 5/12/23, at 9:30 am, Licensing Program Analyst (LPA), Vicky Williamson conducted an unannounced annual required inspection and met with Director, Mary Foust and Assistant Christine Evans. LPA disclosed the purpose of the inspection and was led on a tour of the facility. Facility hours of operation are Monday, Wednesday and Friday 9:00am – 1:00pm. Facility is closed on Tuesday and Thursdays. There were 11 children in Room 4 with Director, Mary Foust. Furniture and equipment are in good condition. A small area of one of the structures on the playground had exposed wood during the time of inspection. Director was reminded that all playground equipment should be in safe condition. Director stated that she understood. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Outside play area has canopies used for shade. The area under high-climbing equipment and slides has cushioning to absorb falls. Toilet and hand-washing equipment are in safe and sanitary operating condition. Floors in the facility are clean and safe. Solid waste storage containers have tight-fitting covers and are in good repair. Disinfectants, cleaning solutions and other hazardous items are made inaccessible. No poisons were observed. Drinking water is available both indoors and outdoors. Facility has a functioning carbon monoxide an smoking detector that met statutory requirements. Drinking water is available both indoors and outdoors. The last fire/disaster drill was conducted and documented 4/24/2023. Director stated there are no bodies of water on the premises. Director stated there are no firearms or ammunition allowed or stored on the premises. The parents bring lunch and snacks. A review of staff records on this date indicates facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or

exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Capacity and limitations as specified on the license are being maintained. Facility maintains a ratio of one teacher supervising no more than 12 children in care. Children are under supervision, including visual supervision, of a teacher at all times. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at off-site activities.The person who signs the child in/out of the facility shall use their full legal signature and record the time of day.

LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening and immunization records. Mandated Reporter training for Staff 1 (S1) (See LIC 811 Confidential Names List) expired on 10/22/21. LPA informed Director to ensure the mandated reporter training is completed once every two years.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA and director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Guardian and California Megan’s Law (www.meganslaw.ca.gov).

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations deficiencies were cited. See LIC 809D for cited deficiencies. A Technical Violation was issued.

An exit interview was conducted with Director, Mary Foust, and a copy of this report, Appeal Rights, LIC 809D and Notice of Site Visit were provided. Notice of Site Visit is required to be posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA observed Notice of Site Visit posted on the bulletin board in the entry area of the facility.
