On 10/29/2021 at 12:00pm, Licensing Program Analyst (LPA) Selina Siao conducted a plan of correction visit to ensure that the citations issued on 10/20/2021 have been corrected. Upon arrival, LPA Siao met with Director Darla Rodriguez in the office and conducted a tour of all three preschool classrooms. The children were observed eating lunch, finishing lunch and getting ready to nap and the children in the two year old class were resting on napping mats. All the children had their mask in their cubbies or next to them at the table while eating and the two year old children had their mask in an individual zip lock bag with their names.

All staff members have the required background clearances and are associated to the facility. LPA cleared the citations issued on 10/20/2021.

Notice of cite visit issued and it must be remain posted for 30 days. Provided appeal rights to facility representative today.
