13 | On 7/17/23, at 9:45 am Licensing Program Analyst( LPA) Annette Sutherland conducted an unannounced complaint inspection regarding the above allegation. LPA met with Director Christine Collins and toured the facility. LPA interviewed staff, reviewed select staff and children files and obtained copies of pertinent documents. Department received a self reported incident as well as a co-complaint that occurred on 7/10/23. It was determined that at least 3 incidents of handling a child in a rough manner have occurred since date of hire for Staff #1, which included pulling a child’s ear and arm. Director was aware of all incidents. S1 received verbal warning. No retraining was provided to S1.
Based on evidence gathered, the above allegation is substantiated. LPA Annette Sutherland informed Facility representative that this report dated 7/17/23 documents 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Annette Sutherland informed the facility representative Christine Collins to provide a copy of this licensing report dated 7/17/23 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
