NARRATIVE
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On July 14, 2021 at 9:00 a.m. Licensing Program Analyst’s (LPA’s) Leilani Curtis and Patrick Ma conducted an unannounced case management inspection to follow up on a self-reported incident that occurred on 6/30/21, wherein a one-year old child (C1) walked out of the facility into the parking lot unsupervised. LPA’s met with Director Christine Collins and proceeded to tour the facility. There were 21 children with 7 staff members present. Appropriate ratio/capacity were observed. Staff members have the required background clearances and are associated to the facility. LPA's interviewed Staff #1 (S1), Staff #2 and Director (D1). Staff #2 was interviewed via telephone from the facility. Prior to arriving at the facility a witness/parent (P1) was interviewed. On 6/30/21 at approximately 4:20 p.m. the Starfish class was leaving the playground to go into the classroom. At approximately the same time a parent (P1) arrived at the outdoor play area to pick up his child. P1 picked up his child and left the playground closing the gate behind him. After P1 strapped his child into his car seat he noticed C1 on the opposite side of the parking lot between two cars. At the time of the incident there were 9 children and three staff members present on the playground. The director states that C1’s father was notified of the incident when he came to pick up his child. On July 1, 2021 the director met with the infant center staff to discuss proper supervision and all staff involved received “written warnings” in their file. The lead staff member involved in the incident is no longer employed at the facility. All facility staff watched the Supervising Children in Childcare Settings video on the Community Care Licensing website. LPA's obtained a copy of a signed declaration/affidavit from staff members confirming that they watched the video. The director states that the wood gate which leads into the playground will be replaced by a self latching metal gate with a security code panel to allow access. See LIC809D for cited deficiency.

An exit interview was conducted with the director and appeal rights (LIC 9058 1/16) were discussed. The director’s signature on this form acknowledges receipt of these rights. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA’s observed the director post notice of site visit.

Per Chapter 3, Division 12, Title 22 of the California Code of Regulations, upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. A copy of the Fact Sheet - AB 633 Child Care Parent Notification Requirements and a copy of LIC 9224 was given to the director.
