On 07/03/2023 at 12:50pm, Licensing Program Analyst (LPA) Selina Siao conduct an annual inspection. Upon arrival, LPA met with Director Kate Yohe and proceeded to tour the facility. The following ratios were observed: In the Frogs 1 Class (3 year old) has 9 children napping on mats supervised by teacher Kristina Brittingham, The Honeybees class (2 year old) had 6 children napping on mats supervised by teacher Vivian Elias and teacher aide Mariah Niber. At the next building Frogs 2/Monsters Class had 9 children napping on cots supervised by teacher aide Khalda Jabo. In the Owls Room there were 16 children that are from the Frogs and Owls class napping on mats and the children were supervised by teacher Sarah O'connel and Salam Abd aka Ban Brikho. Facility is within staffing ratio. All required notices, forms and license were posted. Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation and drinking water are available for children. Storage cubbies are readily available, and room accommodates class size. Napping equipment consists of mats and cots which are kept in the classrooms and each mat/cot has its own sheet or covering. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Food service area consists of a kitchen which is clean and free of hazards. Menu is posted. Adequate food is available for meals and snacks. Cleaning supplies are kept out of reach of children. Outdoor play area is a fenced playground with sand for cushioning under and around the climbing structure. Climbing structure and slides are securely fixed to the ground. The two playground has either a big tree or a large canopy used for shade. Equipment is age appropriate for the preschool age children. Drinking water is available for the children inside and outside. Facility uses electronic sign in and out system. First aid supplies and medication policy and storage was reviewed, all areas are within compliance.

Isolation area is the Director's office. Personnel and client records were reviewed and most areas are in compliance. All staff members has a valid online mandated reporter training certificates. Reporting requirements was also reviewed. All personnel have required criminal record and child abuse index clearances or exemptions. LPA reviewed the Emergency Disaster Plan and the last fire drill was conducted on 04/19/2023. Facility has an operating carbon monoxide detector located on the ceiling in the Frogs 1 class near the kitchen door. Facility has at least one staff member that has a valid EMSA approved CPR/FA when children are in care.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Child Care Providers can now sign up for Quarterly Updates and PINS through the DSS website at https://cdss.ca.gov/inforesources/community-care-licensing/subscribe. LPA discussed California Megan's Law with provider and advised her to go on the website at www.meganslaw.ca.gov.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Refer to the next page LIC 9102 technical advisory notes for areas that needs corrections. An exit interview was conducted, and appeal rights were provided.

All unusual incident reports shall be submitted to Licensing office via email at SDIncidentReports@dss.ca.gov or via fax at (619)767-2203.

A notice of site visit was given and must remain posted for 30 days. Failure to keep notice posted will result in a civil penalty of $100.00.
