Licensing Program Analyst (LPA), Marie Hernandez conducted the unannounced Required inspection. LPA met with the Facility Representative, Gini Andrade and the Director Lisa Daniels. LPA was given access to inspect the facility. The Representative Ms. Andrade accompanied LPA on the tour of the facility. LPA inspected the classrooms for compliance. In room #1, there are nine children with one staff, in classroom #2, there are eight children with one staff, in the classroom #3, there are eight children with one staff and room #4, there are ten children with one staff. The facility has maintained the appropriate ratios and capacity during the inspection. The staff are providing the appropriate care & visual supervision of children during the inspection. There are no bodies of water and there are fire arms/weapons on the premises during the inspection. The facility is ensuring that all children are being visually supervised by a teacher during the inspection. The facility is adhering to a ratio of one teacher supervising no more than 12 children in attendance during the inspection. The facility is operating within the conditions and limitations specified on the license during the inspection. The disinfectants, cleaning solutions, poisons and other dangerous items are inaccessible to children during the inspection. The storage areas for poisons are locked during the inspection. All materials and surfaces accessible to children are toxic free. The furniture and supplies are age appropriate. The rooms have adequate lighting, and ventilation during the inspection. The floors are found to be clean and safe during the inspection. The drinking water is readily accessible inside and outside for the children during the inspection. The restrooms are maintained with operational toilets and faucets with appropriate temperature during the inspection.

Hand soap, paper towels and toilet paper are available during the inspection. The restrooms are lighted and ventilated during the inspection. The food service area is clean and free of hazards. The menu is posted. There is an adequate supply of food items available for meals and snacks. The outdoor high climbing play structure with slide has sufficient cushioning material to support a child's fall. The facility has maintained the fire/disaster drills; the last drill was conducted on 10/10/2019. The cleaning supplies are kept separate from food. All foods/beverages capable of rapid spoiling are stored in covered containers. The facility appears to be free of insects and rodents during the inspection. There is an operational carbon monoxide detector during the visit. There is adequate shaded rest area for children during the visit. The toys are age appropriate during the visit. The grounds are free of debris or potential hazards during the visit. The sign-in/out sheets were reviewed. A review of staff's criminal record clearances on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions during the visit. The Lead Exposure handout was discussed and provided.

Due to time constraints, LPA will have to return to the facility to review staff and children's records.

The following deficiency was cited today. An exit interview was conducted and a copy of the report, Notice of Site Visit and the Appeal Rights were provided to the Director. LPA observed the Director post the Notice of Site Visit in a prominent place. The Director states it is understood that this notice must be posted for 30 days.

Sign In and Sign Out - The person who signs the child in/out shall use his/her full legal signature and shall record the time of day. The person who brings the child to, and removes the child from, the center shall sign the child in/out. A person who removes the child from the center during the day, and returns the child to the center the same day, shall sign the child in/out.

This requirement was not met as evidenced by LPA's review of the sign in/out sheets. LPA observed that on 11/6/2019, 11/5/2019, & 11/4/2019 child #1, child #2 & child #3 were not signed out of the facility. On 11/01/2019, child #4 was not signed in but was present at the facility on that day. This poses a potential health and safety risk to children.
