On 2/17/23 at 11:00 AM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced proof of corrections (POC) visit to the facility. Upon arrival, LPA was greeted by Director Celida Banuelos and toured the facility. Census was 26 infants/toddlers as follows:
  • Infant room had 7 infants with staff members: Gabby Norris, Mai Yang and Jenavee Ogo
  • Mobile Infant room had 7 infants with staff members: Sara Said and Kenjanai Doucet
  • Toddler room had 12 toddlers with staff members: Jazmin Valencia, Sabrina Smith and Nicole Cortes
Prior to the visit on 2/13/23, LPA received a corrections policy via e-mail from the Director that stated the sensory table should be inspected prior to use and no rubber mulch should be used for pouring or in sensory table. The document also stated that rubber mulch/wood chips are ONLY for fall zone and should not be used for activities, pouring or play. Staff members were made aware of the updated policy and signed the form.

During the visit, LPA observed the toddler playground that has the rubber mulch for the playground surface. LPA observed new equipment and materials for the children to play with. LPA also observed that all cups, bowls and pouring devices were removed from the playground. LPA interviewed Staff members #2 (S2) & #3 (S3) on the playground. Staff members were aware of the new policy. Staff #2 stated that they received new balls and that more equipment is being ordered. Director stated she has purchased additional equipment and materials for the playground as well and that a proposal has been submitted to "revamp the toddler playground" and possibly replace the rubber mulch with another suitable cushioning material. On the mobile infant playground that does not have rubber mulch surface, (it has a green turf surface), LPA observed Staff #4 asking Child #1 (C1) what she had in her mouth and swiped her mouth with a gloved hand. The infant had two pieces of rubber mulch in her mouth which were removed and thrown onto the ground. LPA asked how the rubber mulch got onto this playground. Staff stated that sometimes the mobile infants use the toddler playground and it gets tracked to the other playground.

Type B deficiency has been corrected and cleared. Notice of clearance was provided to Director.

Exit interview conducted and report was reviewed with facility representative Director Celida Banuelos. A notice of site visit was given and must remain posted for 30 days.
